What to Expect

To ensure the highest quality of service, we employ a team approach that incorporates the perspectives of individuals, their families, and a group of professionals, including neuropsychologists, behavioral psychologists, psychiatrists, primary care physicians, and mental health counselors.

Upon admission, we conduct a comprehensive assessment within the first 90 days. This assessment serves as the basis for developing an Individualized Service Plan (ISP) that addresses the specific needs identified by the client, their family members, and the rest of the service team. To track progress, monthly team meetings are held to review ISP objectives. Any progress, problems, or modifications to the ISP are meticulously recorded in Monthly Progress Notes. In addition, monthly outcome measurements are conducted.

Our Mission

Our mission is to improve the quality of life of individuals who face the lifelong challenges of cognitive impairment, behavioral issues. We strive to conduct our services in a supportive and nurturing environment that promotes each individual’s stability, dignity, achievement of personal life goals, and movement toward recovery, personal independence, community integration, and self-advocacy.
